Parkdale Community Health Centre (PCHC) is situated in a vibrant, multi-cultural, downtown neighbourhood. We work with the community, in all its diversity, to address its health-related needs through community development and action, the delivery of primary health care, counselling, advocacy and health promotion. We give priority to individuals and groups who traditionally encounter barriers to high quality health care services, including refugees and immigrants, people who are Lesbian, Gay, Bisexual, Transsexual and Transgender, people who live in poverty, psychiatric survivors, women, seniors and people who live on or near the street.

Clinical Dietitian – Maternity Leave
Responsibilities:
1- Performs nutritional assessments; develops client focused nutritional plans, health education, counseling, referrals.
2- Documents care; participates in chart reviews and case conferences.
3- Participates in the development, implementation, monitoring and evaluation of treatment, education and health promotion programs and services for individual clients, families and the community.
4- Maintains liaison with physicians, nurses and other providers concerning problems affecting clients’ food habits and nutritional well being.
5- Keeps appropriate statistical records

Qualifications:
1- Undergraduate degree in dietetics or nutrition or equivalent.
2- Completion of dietetic internship recognized by the Dietitians of Canada or equivalent approved combination of education and experience in nutrition.
3- Registration with the College Dietitians of Ontario.
4- Three to five years of experience in clinical diet counseling and community nutrition.
5- Demonstrated knowledge of the community based health care model.
6- Demonstrated commitment to principles of anti-racism and anti-oppression; and ability to work with diverse staff team and clients.
7- Demonstrated understanding of the impact of the social determinants of health on people’s ability to maintain a healthy diet.
8- Excellent oral and written communication skills
9- Ability to speak another language is an asset.
